If you are a member of a group practice* the best way to ensure your group is being rewarded for all your purchases is to register your group using this form. To complete the form, please forward this page to your group administrator who can fill out the form and send to KaVoKerrPreferred@KaVoKerr.com.
Once the form is submitted, the entire group will receive an invite with further instructions on how to complete your registration. Then you can all rest easy knowing that you are all earning rewards together.
*Group Practices consists of offices in a corporate structure that have 3 or more locations.
